Notes: The data from the Center for Medciare & Medicaid (CMS) Statistical Suppplement years 2008-2013, includes data from years 2006-2012. CMS combined data for years 2006& 2007 in 2008 report.CMS produced the Statistical Supplement reports the year after data was collected. Data used for years 2013-2016 came from CMS Program Statistics, which has replaced the Medicare and Medicaid Statistical Supplement.
